Informed Consent for Therapy Services

By engaging in services with Illuminate Counseling, you acknowledge and agree to the following:

• Nature of Services: I provide trauma-informed psychotherapy, EMDR intensives, somatic healing, and group therapy. I may also incorporate holistic and body-based tools with your consent.
• Scope of Practice: I am a licensed psychotherapist in Wisconsin and Ohio. I do not provide services outside of my licensure scope.
• Confidentiality: All sessions are confidential, with legal exceptions (e.g., risk of harm to self or others, court order, mandated reporting).
• Cancellation Policy: Please provide at least 24 hours’ notice to cancel or reschedule, or a fee may apply.
• Fees: Private pay rates vary by service. See the Good Faith Estimate below for details.
• You may pause or end therapy at any time. Consent is ongoing, and you may ask questions at any point.
