Terms and Conditions

These Terms and Conditions ("Agreement") govern the therapy intensive session services provided by Illuminate Counseling, LLC, a licensed mental health professional in the State of Wisconsin ("Therapist"). By participating in the therapy intensive session ("Session"), you (the "Client") acknowledge and agree to the following terms:

1. Session Overview

The therapy intensive session is a focused therapeutic experience designed to address specific mental health goals or concerns in an extended format. The Session may last multiple hours or take place over one or more days, depending on the agreement between the Client and Therapist. The nature of the Session will be outlined in the Client’s individualized treatment plan.

2. Scheduling and Payment

  • The Session(s) will be scheduled in advance by mutual agreement.

  • Full payment is due at the time of the Session. Fees for the Session are as follows: 3-Hour Intensive $700, Full Day Intensive $1700, 3-Day Retreat $5500.

  • Payments can be made via major credit cards, Apply Pay, Affirm, Afterpay, PayPal

  • Cancellations must be made at least 48 hours in advance and appropriate reschedule will be offered. No shows will result in no refund.

3. Confidentiality

  • All personal information and communication shared during the Session will be kept confidential, except as required by law or with your consent.

  • Therapist is obligated to disclose information if there is a concern for the safety of the Client or others, or in cases of suspected abuse, neglect, or other legally mandated situations.

  • The Client acknowledges that confidentiality cannot be guaranteed in group settings or online therapy platforms (if applicable), and that sharing information with others during such settings may compromise confidentiality.

4. Therapeutic Process

  • The Client acknowledges that therapy is a collaborative process, and outcomes are not guaranteed.

  • The Client understands that progress in therapy can take time and that therapy intensives may involve confronting difficult emotions or experiences.

  • The Client agrees to actively participate in the therapeutic process, including completing any assignments or activities as directed by the Therapist.

5. Limitations of Services

  • Therapy intensive sessions are not intended as emergency or crisis intervention services. If the Client is in need of immediate assistance or in crisis, they are encouraged to contact a mental health crisis hotline or seek emergency services.

  • The Therapist will not provide medication management, and any pharmaceutical recommendations or treatments must be handled by a licensed physician or psychiatrist.

6. Client Responsibilities

  • The Client agrees to provide accurate and complete information regarding their mental health history and current status, which is necessary for effective treatment planning.

  • The Client agrees to notify the Therapist of any changes to their health status or emergency situations that could impact the Session.

  • The Client must be in a mental and physical condition to participate in the Session. If the Client feels unwell, the Session may be rescheduled.

7. Consent to Treatment

By engaging in a therapy intensive session, the Client provides informed consent to receive treatment. The Client acknowledges they have had the opportunity to ask questions, understand the treatment being offered, and agree to the potential risks and benefits of therapy.

8. Termination of Services

Either party may terminate the therapeutic relationship at any time. If the Client wishes to discontinue therapy, they are encouraged to discuss this decision with the Therapist to ensure proper closure. In the case of termination by the Therapist, a referral may be provided to another mental health professional.

9. Dispute Resolution

Any disputes arising out of or related to these Terms and Conditions shall be resolved through informal negotiation. If the dispute cannot be resolved informally, the parties agree to submit to mediation or arbitration under the laws of the State of Wisconsin.

10. Acknowledgment of Risks

The Client understands that therapy is not without risks, and although therapists strive to provide effective and ethical care, there may be emotional or psychological distress that arises as part of the therapeutic process. The Client agrees to engage in therapy with a full understanding of these potential risks.

11. Compliance with Laws

The Therapist and Client agree to comply with all applicable federal and state laws, including but not limited to the Health Insurance Portability and Accountability Act (HIPAA) and Wisconsin’s mental health confidentiality laws.

12. Limitation of Liability

The Therapist will make every effort to provide professional and effective therapy; however, the Client agrees that the Therapist shall not be held liable for any indirect, incidental, or consequential damages arising from the therapy intensive session.

13. Amendments

These Terms and Conditions may be amended or modified in writing by mutual agreement between the Therapist and Client. Any such modifications will be provided to the Client in writing.

By signing this Agreement, the Client acknowledges that they have read, understand, and agree to the Terms and Conditions outlined above.
